The Minnesota Department of Health (MDH) is seeking proposals to prevent cannabis and other substance use among Minnesota youth through the Communities That Care (CTC) prevention program. This community-based system has been proven to reduce youth health and behavior problems by helping local coalitions develop targeted prevention strategies.
The CTC program follows a five-phase process to:
Establish a community prevention coalition.
Conduct a community youth survey.
Develop a community action plan.
Implement and evaluate prevention strategies.
Prevent youth substance use (alcohol, cannabis, commercial tobacco, etc.).
Reduce risk factors and strengthen protective factors at a community-wide level.
Address related health and social issues, including mental health, violence, and academic performance.
Fund up to 12 communities to implement the CTC prevention system.
Organizations must demonstrate the capacity to lead local coalitions and implement prevention strategies
Must have the ability to evaluate progress based on measurable outcomes.
Estimated Award Amounts:
Maximum: $150,000
Minimum: $100,000
